On Tuesday, September 15, our team packed up the table, the banners, and the materials and headed to Bakersfield College for the Kern County PATH Collaborative Planning and Implementation (CPI) in-person meeting.

What PATH CPI is, and why we show up

The PATH Collaborative Planning and Implementation initiative is part of California's CalAIM transformation. It funds regional efforts that bring together managed care plans, providers, community-based organizations, county agencies, hospitals, and tribal partners — all with the goal of making sure Enhanced Care Management and Community Supports actually reach the Medi-Cal members who qualify for them.

That sounds procedural on paper. In practice, it's about closing the distance between a person who needs help and a service that already exists to help them. Kern County is large and spread out. Someone in Delano and someone in Tehachapi may be eligible for the exact same support and have very different odds of ever hearing about it. Collaboratives like this one are how that gap gets narrowed — through clearer referral pathways, stronger partnerships, and honest conversation about what isn't working yet.

So when there's a chance to be in the room, we take it.

At our table

Vista Care staff hosted a table throughout the event, sharing information about our Community Health Worker services with partner organizations, campus visitors, and community members who stopped by.

The questions we get at events like this are usually the same ones we hear on the phone: What does a Community Health Worker actually do? Who qualifies? Does this cost anything?

The short answers, for anyone who missed us:

Health education — understanding a diagnosis, a medication, or a care plan in plain language
Health navigation — help getting through the system: appointments, paperwork, coverage, transportation
Screening and connection to services — identifying needs and linking members to resources that address them
Individual support and advocacy — someone in your corner who knows how the system works

Vista Care's CHW services are available at no cost to eligible Medi-Cal members in Kern County.

Why being there in person matters

Most of our work happens one person at a time — in living rooms, waiting rooms, and parking lots. That's how it should be. But it also means the day-to-day can feel small, even when it adds up to something much larger.

Days like Tuesday put that larger picture back in view. Being outside, at a table, alongside other organizations serving the same neighborhoods we serve, is a reminder that none of this happens in isolation. It's also the simplest form of access we can offer: someone can walk up, ask a question, and get a real answer — no appointment, no login, no wait time.

We love being out in the community. Not as a slogan, but as the reason this work is worth doing.
